Accommodation/Tenancy
Support to help participants obtain or maintain appropriate accommodation, including assistance with tenancy obligations, applications, and understanding rental agreements, in line with individual goals.
Assist-Personal Activities
Support with daily personal tasks such as showering, dressing, grooming, toileting, and other personal care routines. Designed to promote dignity, independence, and daily living skills in the participant’s home or community.
Assist-Travel/Transport
Services that support participants to access the community, education, employment, and daily activities by providing safe and accessible transport options based on assessed needs.
Daily Tasks/Shared Living
Assistance with daily personal tasks within a shared living environment (e.g., group or supported independent living). Focuses on developing independent living skills through support with routines and household management.
Innov Community Participation
Specialised supports that encourage participants to engage in community, social and recreational activities. Aimed at fostering social connections and building confidence through tailored, capacity-building programs.
Development-Life Skills- Household Tasks
Capacity-building support to enhance a participant’s ability to live independently. Includes skills training in areas such as budgeting, cooking, travel training, personal hygiene, and household management.
Participate Community
Supports that help participants actively engage in community life, including attending events, joining groups, and participating in social and recreational activities, fostering inclusion and personal development.
Support to help participants obtain or maintain appropriate accommodation, including assistance with tenancy obligations, applications, and understanding rental agreements, in line with individual goals.
